How to complete your enrollment forms
Working through the form itself, and the four things that stop it submitting.
Where the form comes from
Your employer will send you a link, or we will send it on their behalf. It opens in a browser. There is nothing to install and no account to create.
If you cannot find the link, ask your HR contact first. They can resend it. We can too, but they can usually do it faster.
Do it in one sitting if you can
A laptop or a desktop is easier than a phone for this, mostly because of how much typing there is. Somewhere quiet with your documents in front of you beats squeezing it in between meetings.
Working through it
- Enter every name exactly as it appears on that person's Social Security card. Not a nickname, not a shortened first name.
- Add each person you are covering in their own section. Do not combine two children into one entry.
- Choose your plan. The plan choices, and what each one costs you out of your paycheck, are in the materials your employer shared. We cannot tell you your cost by email, because your employer sets how much of it they pay.
- If you do not want the coverage, say so on the form. That is the waiver, and skipping it is not the same thing.
- Sign at the end. An unsigned form does not get submitted, and nobody is notified that it is sitting there.
Four reasons a form will not submit
If the submit button is doing nothing, it is almost always one of these. The form usually shows a message at the top, but it is easy to miss on a phone.
- A Social Security number is missing for you or for someone you added.
- A date is in a format the form did not accept. Use month, day, then the four digit year.
- A dependent was added without a relationship selected.
- The signature at the end has not been completed.
Deadlines
Enrollment windows close, and the date is set by your employer and the carrier rather than by us. A form that arrives after the window usually means waiting for the next open enrollment, unless you have had a qualifying life event.
If you are going to miss the date, tell your HR contact before it passes rather than after. There is more that can be done beforehand.
If you get stuck
Email support@bindlyhealth.com and tell us which step you are on and what the screen says. A person reads every message. If your question is about your health or your care rather than the paperwork, call the benefits desk at (623) 257-7625 instead.
