What to have ready before you start

The information the enrollment form asks for, gathered before you open it.

Give yourself fifteen minutes and gather these first

The enrollment form is short, but it asks for information most people do not have memorized. If you start without it you will end up saving a half finished form and coming back, which is where most missed deadlines come from.

  • Your legal name, spelled exactly as it appears on your Social Security card.
  • Your Social Security number.
  • Your date of birth.
  • Your home address, and your mailing address if it is different.
  • The date your coverage should start. Your employer or your HR contact has this.
  • Details of any coverage you have right now, including who it is through and the date it ends.

If you are covering anyone else

A spouse, a domestic partner, or a child you are enrolling each needs their own set of the same details. Missing one is the single most common reason a form comes back.

  • Legal name as it appears on their Social Security card.
  • Social Security number.
  • Date of birth.
  • Their relationship to you, so the carrier knows whether they are a spouse or a child.

Why the form asks for Social Security numbers

The carrier uses them to tell people apart, and it has to report who was covered and for which months. Without a Social Security number for everyone on the form, the carrier cannot finish setting up the coverage.

Put them in the enrollment form itself. Please do not email them to us, and please do not put them in a text message. The form is the right place for them and it is the only place we ask for them.

If you are not taking the coverage

You still have to tell us, in writing, on a short form. That is called a waiver and it takes about a minute. It is covered in its own article.

Questions we did not answer

Email support@bindlyhealth.com. Tell us which step you are on and what the screen says. A person reads every message.

Anything about your health or your care goes to our benefits desk at (623) 257-7625 rather than to email.
